Soaring Minds does not currently file insurance, however, I can provide you with a “Superbill” that you may submit to your insurance company following the completion of the evaluation. It is advised that you call your insurance company before scheduling an evaluation to see if they will provide you with any reimbursement for services.
Please be aware that evaluations for Learning Disabilities, learning / academic problems, academic / testing accommodations (e.g. SAT/ACT, college, LSAT, MCAT, GRE), other academic issues (e.g., 504 Plan, IEP), disability, and anything legally-based ARE NOT covered by insurance. Medical insurances deem evaluations for such issues as "not medically necessary."
Please contact me for information and pricing for other assessments, such as private school admissions, kindergarten readiness assessments, or medically necessary ADHD evaluations, as pricing may be different according to needs of evaluation components. Below are estimates based on evaluation packages. The fee for your evaluation will be tailored to your specific needs.

ADHD - Child/Teen

Testing
Assessment may include interviews, record review, in person testing, and the completion of parent and teacher rating scales.
50% of the balance is due in order to schedule the testing date.

The final balance is due before this meeting is held.
